Few medical insurance plans pay 100 percent of the expenses for hospital and medical care. Even qualified health plans may require an individual to cover deductibles, co-pays, co-insurance and other out-of-pocket expenses – which can add up to thousands of dollars a year. Chubb Accident & Health’s GAP Supplement Insurance Plan is specifically designed to fill the gaps of an underlying major medical plan by reimbursing an individual’s covered out-of-pocket expenses.

Highlights

  • Supplements an employer’s major medical plan by reimbursing individuals for covered out-of-pocket expenses for which they are responsible for.
  • Hospital benefit reimburses deductibles, coinsurance and other eligible out-of-pocket expenses for covered benefits incurred during an inpatient hospitalization.
  • Optional outpatient benefit reimburses eligible out-of-pocket expenses incurred for hospital emergency room visits (not admitted) and use of outpatient surgical, diagnostic testing and other similar facilities licensed to provide outpatient treatment.
  • Optional ambulance benefit reimburses eligible out-of-pocket expenses incurred for transportation provided by a licensed ambulance company to the nearest hospital.
  • Coverage is guaranteed issue.
  • Individuals covered under this plan must also be covered by an underlying major medical plan.

Minimums:

  • Plan is only available to employer groups that have a major medical plan available for employees.
  • Eligibility rules are flexible in order to coordinate with a major medical plan’s eligibility rules; coverage is also available to dependents of employees who are covered under the major medical plan.
  • Minimum of 30 days of active service required (employer may choose to lengthen the number of days).
  • Employees must work at least 15 hours per week.
  • At least 15 employees must participate in the plan.
